2. Enter your personal details: name, last name, gender, and date of birth.
3. Click “Next” to continue.
Step 4: Enter Your Address Details

– Sub-district
– Village/Town
– Pincode
– Building number
– House number
– Street name

Step 5: Provide Aadhaar and Nationality Details

Confirm that the child’s address is “In India” and fill in the address details. If the child’s address matches the parents’, select the “Copy Parent’s Address” option.
Step 7: Specify Birthplace
Choose the specific birthplace, such as a hospital. Then, select the state, district, sub-district, and town/village. If it’s a town, fill in the ward details.
Step 8: Select Registration Unit
Choose the “Registration Unit” where the birth occurred. If “Other” is selected, you’ll need to manually enter the hospital’s name.
Filling Out Statistical Information
Enter details such as:
– Parent addresses, if different from one another
– Religion, father’s education level, and occupation
Health Information
Include the mother’s age when filling out the form and her age at the time of birth. Add the number of children she has, including the newborn.
Delivery Details
